Functions/Duties of Position
Spanish Speaking Nurse Case Manager will provide comprehensive case management services to HIV
clients and their families through the continuum of HIV related care. Key services include health assessment, coordination of services, transition planning, and education in conjunction with an interdisciplinary team.
Job Requirements
Required:
Current, unencumbered Oregon State nursing license.
BSN required.
Current OHSU employees: Depending on unit need, internal candidates who are a Diploma RN or who have an Associate's degree in Nursing will be considered. BSN preferred.
Must have graduated from a Commission on Collegiate Nursing Education (CCNE) accredited program, or National League for Nursing (NLN) accredited program for nurse educator roles.
BLS certification issued by the American Heart Association or the Military Training Network.
Bilingual in Spanish/English.
Must have the ability to problem solve and work with extremely complex clients who present with multiple diagnoses and social problems.
Understanding of the role of the nurse and the scope of practice in Oregon.
Needs to have understanding of cultural factors that influence service access and delivery.
Must have excellent written and oral communication skills.
Must have working knowledge of computer operations and EHR.
Preferred
A minimum of two (2) years of applicable HIV clinical experience in community health and/or case management.
Knowledge of HIV disease and its related diseases and infections
Additional Details
Working Conditions: Work involves meetings, home visits and traveling to numerous medical clinics. Travel to Multnomah, Clackamas, Columbia and Washington Counties.
Physical Demands & Equipment Usage: Ability to drive to and from various locations in a day and provide home/office visits
Orientation: Three (3) to five (5) weeks
