Summary:
This position is responsible for providing information, referral, crisis and short-term intervention and assistance in the coordination of service delivery to clients who are elderly and/or younger adults with disabilities. This position also is responsible for maintaining office data systems and completing client intakes.

Entry Wage: $13.77 - $15.99/hr

Responsibilities:
Greet consumers who walk-in to the office requesting referrals, research referral requests and provide information, deescalate challenging situations, communicate in a positive and professional manner with all individuals;
Maintain and update the central resource file in order to help seniors with issues such as, but not limited to, housing, caregiving, medical equipment, rental assistance, energy assistance, etc. The I & A Specialist needs to be familiar with the resources available in these areas, referring seniors and people with disabilities to the appropriate services.
Receive and respond to initial requests for services, record pertinent information, make referrals to appropriate internal programs and/or outside agencies; triage incoming calls from a variety of referral sources;
Act as a screener and intake worker for the Multicultural Services programs; maintain on-going contact with key staff of agencies to which clients are frequently referred; maintain open communication with program advocates regarding the referral process and service needs of newly referred clients;
Act as a liaison with, coordinate and schedule Senior Health Insurance Benefits Assistance (SHIBA) Program, Senior Law Project and AARP tax assistance appointments, maintain excel spreadsheets with appointments and upcoming months' schedules;
Responsible for payment of Home Care Worker vouchers and maintenance of accurate records in Mainframe and Oregon Access databases;
Maintain updated directory of language interpreters and culturally specific programs and act as the liaison with the county staff who pay for interpreter services;
Share responsibility for center coverage, including crisis intervention and reception;
Complete accurate, legible and timely documentation of services delivered in accordance with Impact NW's management information system;
Complete ongoing related reports for Program Supervisor, ADSD and other funders according to timelines as required;
Meet client service standards set forth by Program Supervisor regarding quantity, type and quality of services to be delivered to clients and volunteers;
Effectively work with individuals of diverse backgrounds.
Completes accurate, legible and timely documentation of transportation services delivered in accordance with Impact NW management information system and ADS;
Attends regular I&A; and Transportation meetings, ADS Quarterly I&A; meetings, attends, all internal staff meetings, and other meetings or trainings as assigned;
Perform other duties as assigned, following timelines and procedures as instructed;
REQUIREMENTS AND
Qualifications:
Any combination of experience and training that would likely meet the following requirements
Associates Degree or equivalent in social work or related field
Minimum of two years' experience in human services, preferably working with low-income seniors and younger adults with disabilities
Experience working with seniors and/or adults with disabilities
Sensitivity and ability to relate to people from diverse lifestyles and cultures
Ability and willingness to work as a team member and support Impact NW's mission and goals
Ability to communicate effectively and professionally, both orally and in writing in a way that supports open, honest communication
Ability to effectively work within a strength-based, integrated family support service delivery model
Demonstrated computer skills (especially data entry) and knowledge of Microsoft Office programs and Internet
Possess good organizational skills and ability to prioritize multiple and varied tasks
Prepare for and pass AIRS test within one year of employment
PREFERRED BUT NOT REQUIRED
Qualifications:
Bilingual in Spanish, Chinese (Mandarin or Cantonese), or Vietnamese
Competency and experience working in two or more cultural communities
