Purpose: (General description and summary)
The Psychiatric Mental Heal Nurse Practitioner (PMHNP) provides assessment and monitoring of patient's psychiatric, medical and social needs, including review of systems, individual service and support plans, and service conclusion planning as a part of a multi-disciplinary team. Provides consultative, diagnostic, and prescriptive services. Assesses and treats emergencies; oversees medication monitoring and administration in compliance with applicable OAR's. Functions as a liaison between residents, physicians, administration, and other healthcare providers.
Accountabilities: (Responsibilities of the job)
Provide consultation services regarding appropriate interventions and service planning to consumers, Service Coordinators, and family members
Monitor the administration of medication to individuals by program staff to ensure that dispensation is appropriate and in compliance with applicable OAR's.
Provide diagnostic services to all consumers on the identified caseload. Request and monitor lab results as needed.
Complete Psychiatric and Mental Health Assessments as needed. Be prepared to complete a minimum of 2 psychiatric assessments per week if required.
Educates residents on medication names, dosages, functions, and side effects (depending on individual Residential Service Plan).
Write prescriptions for all consumers on the identified caseload and monitor/manage side effects as needed. Approve refills, coordinate with pharmacy, and maintain current orders in TIER.
Follows guidelines in the medication policy. Constantly alert to changes in medication.
Follows up on identified health concerns of individual residents.
Communicates regularly with services and support team members regarding individual's health status.
Assist in the development, implementation and monitoring of emergency services and protocols for individual care.
Responsible for documenting all critical incidents in a timely manner and notifying supervisor immediately of any critical incident.
Ensures individuals are receiving highest quality care.
Assists in managing medical and pharmaceutical supplies.
Provides health education to residents, including recovery skills to prevent a relapse of mental health symptoms. Some groups will include exercise and relaxation techniques to cope with symptoms of depression and anxiety.
Meets individually with residents on a self-medication program to review current medications.
May be asked to facilitate New Narrative staff medication trainings.
Delegates nursing tasks when needed and as appropriate.
Other general expectations:
Attend and positively participate in required meetings.
Collaborate with the clinical and residential staffs.
Treat clients with respect and with trauma-informed care.
Participate in ongoing training in the area of mental illness.
Follow all corporate polices, procedures, and protocols.
Comply with all Luke-Dorf safety rules and regulation. Report to your Supervisor all workplace hazards, injuries, and illnesses, no matter how minor.
Adhere to confidentiality, Professional Ethics, and Dual Relationships per policy and Administrative rule.
Maintain a clean workspace.
Seek out and attend trainings and in-service opportunities (8 hours required per anniversary date).
Attend initial complete CPI training and annual updates.
Maintain current Oregon or Washington Drivers License.
Follow through on all directives and instructions conveyed by the Administrator.
Maintain the security of all corporate interests (i.e. security of keys, files, facilities, etc.)
Other tasks as assigned by Supervisor.
This position is not on-call per se, but is expected to respond to occasional after hours consultation requests from the after hours on-call clinician.

Minimum
Qualifications:
Must have at least 1 year working experience as a Nurse Practioner required.
This position is a hybrid role and requires on site work, with 1-2 days working from home.

Knowledge of medication and correct administration, reducing the potential for medication errors.
Pay Range: This position is offering an hourly rate of $85.00/hr - $105/hr
Education/Certificates/Licenses/Registration:
Currently Licensed as a PMHNP in the State of OregonEstimated Salary: $20 to $28 per hour based on qualifications.
