Home Health LPN for North team area, M-F, no weekends, no holidays, OR and WA license required. Home Health experience required, EPIC/Health Connect experience preferred Hollee Arreola

Responsibilities of the Registered Nurse (RN) Home Health
- Provide health care services in patients residences
- Transport and accompany patients to doctors office or to hospital
- Provide companionship and basic emotional or psychological support
- Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
- Assist with clients personal care activities
- Provide companionship and basic emotional or psychological support
Requirements of the Registered Nurse (RN) Home Health
- Registered Nurse with current license to practice in the applicable state.
- 2 years of experience working as a Registered Nurse
- Proven working experience as home health aide
- Home Care RN experience, strong assessment, teaching, communication, customer service, symptom management, care planning and case management skills IV cert:
Infusion pumps (CADD) BLS for healthcare providers . Must have valid driver s license, ability and willingness to drive in most weather. Flexible, reliable Experience documenting in Home Care EMS system Weekend rotation with other staff preferred
